Does anyone have recommendations for survey software for web sites? Are
there any open source survey scripts?

Are you looking for a pole feature by any chance? PHP-Nuke (GPL) delivers it
'out of the box'. For point-by-point survey, you should be able to re-use
(READ: nick) some existing implementation of a form. I can think of a few
example sites.
